Botox in Spain: Who Can Administer It?
Before booking treatment abroad, you want to know who is legally permitted to perform the procedure. Spanish law names five statutes relevant to this question, and only a doctor registered with a provincial medical college meets all five requirements.
What Spanish law says about who can inject
Cosmetic botulinum toxin in Spain is regulated as a medical act. That means only a médico colegiado, a licensed, college-registered doctor, may perform it. The facility itself needs a specific health-center license.
The following legal and regulatory provisions set out these requirements:
- Ley Sara. In force from July 1, 2024, this law reclassifies aesthetic procedures as medical acts under continuous physician responsibility.
- Royal Decree 1277/2003. Cosmetic Botox is filed under Healthcare Unit U.48 and can only be performed with a licensed doctor physically present.
- AEMPS Circular 02/2010. Dentists may inject only for dental and perioral therapeutic conditions. The Annali di Stomatologia review confirms that cosmetic upper-face Botox by a dentist is not legal in Spain.
- Article 403 of the Criminal Code. Injecting without a valid medical license, called intrusismo profesional, carries up to two years in prison.
Nurses, beauticians, hairdressers, and spa staff are outside the license in all of these statutes.
How to Verify Your Botox Injector and Clinic in Spain
Verification takes about five minutes using two free public registries. Both are open to anyone and require no login.
Verify the doctor: CGCOM's public registry
Every practicing physician in Spain is registered with one of 52 provincial medical colleges. These colleges are coordinated at the national level by the Consejo General de Colegios Oficiales de Médicos, or CGCOM. A colegiado number is a public credential, checkable by name on the free CGCOM Registro Central.
Verify the clinic: REGCESS and international accreditation
The clinic itself must hold a U.47 or U.48 health-center license. That license is entered in REGCESS, the Registro General de Centros, Servicios y Establecimientos Sanitarios. It is the Spanish state register of medical facilities.

The Spanish botulinum toxin market offers several branded products. When a clinic gives you a price based on the number of units, ask which brand those units refer to. Unit measurements vary between products, so 50 units of one brand cannot be directly compared with 50 units of another.

Botox and Vistabel use onabotulinumtoxinA units, while Dysport and Azzalure use Speywood units. A commonly used rough conversion is 1 Botox unit to about 2.5 Speywood units, so 50 Botox units is roughly equivalent to 125 Speywood units.

Traditional Botox arrives as a dry powder, and the doctor reconstitutes it with sterile saline in the room. Ready-to-use liquid formulations arrive pre-mixed, which reduces preparation-related variability.

Before treatment, the doctor takes a medical history and checks for any factors that may affect the treatment plan. Follow-up care also forms an important part of the process, so you know what to expect in the days after your injection.
What the doctor screens for
A Spanish physician screens for a short list of conditions where botulinum toxin isn't the right choice:
- Neuromuscular disease: Myasthenia Gravis, Lambert-Eaton syndrome, or ALS.
- Pregnancy and breastfeeding.
- An active skin infection at the injection site.
- A previous hypersensitivity reaction to botulinum toxin.
Two other situations call for a slower plan rather than a firm no. Pre-existing eyelid or brow ptosis is one. Anticoagulant or high-dose NSAID therapy is the other, because bruising risk runs higher.
Common botox side effects
Across 1,437 cosmetic Botox adverse-event reports in the FDA post-market registry, most were classified as non-serious. The main reported issues were:
- Lack of effect: About 63% of non-serious reports, often linked to dosing or dilution.
- Injection-site reactions: About 19%, including redness, bruising, or mild swelling that usually settles within days.
- Temporary eyelid or brow ptosis: About 2.5% of patients – it usually wears off within 3–4 weeks, rarely up to 3 months. The risk is under 1% with experienced injectors.
A few other details are useful to know before treatment. Tell your doctor about aminoglycoside antibiotics such as gentamicin, muscle relaxants, or anticholinergic medicines, as these can increase the effect of botulinum toxin. High daily green tea intake may also affect how long the results last. Leaving at least three months between treatments can help reduce the chance of an immune response that affects future injections. Some long-term users may also discuss complexing-protein-free products with their doctor.
